Policies and Procedures Descriptions
Accession Questionnaire – Completed by the donor (or the Curator if the donor is unable). Provides provenance and other background information about the donation. Information recorded in database by Registrar or Assistant Registrar and hard copy goes in the accession file. Application For Access To Collections - This form should be completed by any non-staff member requesting access to the collection. The reverse side lists the conditions of access. See Conditions Governing Access To Collections. Conditions Governing Reproduction of Works of Art – Conditions that go on the back of the application for access to collections. Application For Rights to Reproduction - This form should be completed by anyone who wishes to reproduce any object, painting, photograph or piece of archival material from the collection and turned in to the Registrar. The first page provides general information regarding the use of the material and the following three pages are a contract between the Museum and the patron regarding use of the material (see Conditions Governing Reproduction of Works of Art) Rights and Reproductions Contract – Contract that gets attached to the application for rights to reproduction. Deed of Gift - A deed of gift is completed by the Registrar and sent to everyone who makes a donation to the Museum as a collection or non-collection object. The status should be designated on the deed of gift and the donor should be made aware that non-collection items are used in the educational programs and do not receive the same level of care as collection items. They are considered expendable. The reverse side lists the Museum's accession and de-accession policies (Collection Policy – Back of Deed of Gift). If all of the items will not fit in the description on the first page, attach additional pages (Deed of Gift 2nd Sheet) Collection Policy – Back of Deed of Gift – Conditions that go on the back of the deed of gift form. Library Research Request Form – Completed by researchers who wish to access the Museum’s library and archives. Permission To Photograph Collections - Completed by the Registrar whenever anyone besides museum staff wishes to photograph collection items. It serves as a contract between the Museum and the photographer guaranteeing that the Museum will own the photograph and the copyright. Procedures for Photographing Collections – Procedures that go on the back of the Permission To Photograph Collections form. Temporary Deposit Receipt (Incoming) - Completed by the Registrar or Assistant Registrar for all items brought in or sent to the Museum to be considered as gifts. This applies to all potential gifts whether or not they are to be accessioned as collection items or non-collection items. It also applies to gifts donated specifically to be sold to benefit the Museum. Also completed for items brought in for purposes of identification, attribution or conservation evaluation. The reverse side lists the conditions for temporary deposits. If all of the items do not fit in the description on the first page attach additional pages (see Temporary Deposit Receipt Incoming 2nd Sheet). See Temporary Deposit Conditions. Temporary Deposit Conditions – Conditions that go on the back of the Temporary Deposit Receipt (Incoming). Photograph Order Form - This form should be used by the Registrar or Librarian when a patron orders an image from the photographic archives. It is mainly used as a means of recording the services requested and it can also be used to generate an invoice. The reverse side lists the photo order policies.
